Thank you for visiting portal.myonelinehealth.com (the“Site”), owned and operated by OneLine Health LLC (referred to as“OneLine Health,” “us,” “our,” “we”). We are committed to respecting privacy rights and concerns and have established and implemented this Privacy Policy to inform users of our software, of what personal information we collect, and how we use, share, and protect such information.Please note that this Privacy Policy does not apply to other personal information gathered or used by your healthcare provider, including Protected Health Information (“PHI”). The processing of PHI is governed by applicable law and the privacy policy of your healthcare provider. 2. Agreement to Terms
If you do not agree with the terms of this Privacy Policy or our Terms of Use, then you should immediately discontinue use of the Site and/or our services without providing us any personally identifiable information.
3. Effective Date and Changes to this Policy
This Privacy Policy is effective as of the date above and will remain in effect until a new policy supersedes it. We may choose to update this Privacy Policy at our discretion, so you should check this page periodically as the terms may change from time-to-time. The most recent version of the Privacy Policy will be reflected by the “Effective” date noted at the top of the page. In the event of a material change to this Privacy Policy, we will provide a conspicuous message either through the Site. 4. Confidentiality of Health Information
Health information that we receive and/or create about you, personally, relating to your past, present, or future health, treatment, or healthcare services, may be “protected health information” under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and the Health Information Technology for Economic and Clinical Health Act (“HITECH Act”). Your health information may also be protected by state privacy and laws and regulations.
OneLine Health understands that health information about you and your health is personal. We support your privacy and make sure that the transmittal and use of your information complies with all laws. In this regard, where applicable, we comply with HIPAA, HITECH, and other relevant state laws and regulations by entering into Business Associate Agreements with the treatment providers for which we provide services to ensure that your protected health information is appropriately safeguarded.

5. The Information We Collect
The types of personal information we collect about you can include:
- Account and Personal information,such as your first and last name, date of birth, email address, password, phone number, billing address, photographs, video recordings, online identifier, Internet Protocol address, unique device identifiers, marketing preferences, and correspondence; and, if you are a patient, your physicians’ or other healthcare providers’ names, your dependents information, and your proxy’s information;
- Professional or employment-related information,if you are a healthcare provider;
- Internet or other electronic network activity information,including browsing history and search history, and information regarding your interaction with the Site;
- Geolocation information;
- Health information about your medical and physical status,if you are a patient, is collected on behalf of your healthcare provider for their electronic health record system, including any records you upload or request to be uploaded from other healthcare providers, and not accessible to us outside of the services we provide to your healthcare provider. Please contact your healthcare provider for more information on how they process your PHI and your rights under HIPAA;
- Characteristics of protected classifications,such as age, race/ethnicity, sexual orientation, gender/gender identity, religion, medical condition/disability/pregnancy, military/veteran status, national origin, genetic information, and marital status; and
- Survey information,when you complete any forms, respond to a survey or poll or questionnaire, we collect the personal information you provide.
We collect such personal information directly from you when you use the Site; submit registration or feedback forms; participate in a survey; or when you otherwise contact us.
You are not required to provide all personal information identified in this Privacy Policy to use our Site or to interact with us, but certain functionality will not be available if you do not provide certain personal information. For example, if you do not provide certain personal information, we may not be able to respond to your requests, perform a transaction with you, or provide you with marketing or other information that we believe you would find valuable.
If you are a patient, we may also collect some personal information about you from third-parties, including your physician or other healthcare provider when they upload content, such as your treatment plan, diagnosis or medical history to your account.
We may aggregate and/or de-identify information collected by the Services or via other means so that the information is not intended to identify you. Our use and disclosure of aggregated and/or de-identified information is not subject to any restrictions under this Privacy Policy, and we may disclose it to others without limitation for any purpose, in accordance with applicable laws and regulations.
7. Children’s Privacy
The Site is not directed to users under the age of 13. We do not knowingly or intentionally gather personal information about visitors who are under the age of 13. If you are aware of, or suspect that, someone under the age of 13 is using the Site without permission, please notify us immediately by contacting us as detailed below.

Please note that unless specified otherwise, cancelling your account alone, will not delete your information.

You play an important role in keeping your information secure. You should not share your username or password for your OneLine Health account with anyone. If you have reason to believe that your account is no longer secure, please contact us immediately as detailed below.

In an attempt to provide increased value to our visitors, we may choose various third-party websites (e.g., your healthcare provider, if you are a patient) to link to within our Site. We have no control over these linked sites, each of which has separate privacy and data collection practices independent from us. As such, we have no responsibility or liability for these independent policies or actions and are not responsible for the privacy practices or the content of any such websites.
